Background:
Application of the nursing process is a vital component of nursing practice.
In this assignment, you will be required to identify assessment data, develop nursing
outcomes, and complete a teaching plan based upon priority nursing needs.
Instructions:
Please read through the assignment instructions thoroughly and review the marking
rubric attached in moodle.
You will be expected to follow APA format (7th edition). Your paper should be 4-6
pages long (excluding title sheet and reference page.)
Your assignment will consist of identification of client situation, assessment of
learning needs and health teaching plan.
We encourage you to use the assignment template provided for part of this
assignment. The template does not need to be APA format. (the rest of the
assignment does)
Please read and choose one of the assignment options. (Scenario 1 or 2)
Scenario 1
Ms. Woods is 85 years old recently had a severe fall resulting in a fracture of the right
arm. She lives alone, her husband passed away 10 years ago. Her daughter Susan
lives across the street and has been coming in daily to help her mother since the
current injury. Mrs. Woods has a longstanding history of hypertension (10 years). She
is prescribed 40 mg of Lasix PO daily.
Mrs. Woods wears glasses and has been diagnosed with Macular degeneration.
Mrs. Woods has come into the clinic for follow-up with her daughter for her injured
right arm.
Her daughter, Susan, reports that her mom has fallen several times in the last 6
months. Susan believes that these falls occur at night and has concerns about her
mother’s safety. Susan reports to you that over the last 3 months, she has noticed
a strong “urine” smell when standing close to her mom, and on the clothes when
doing her mom’s laundry.
Susan reports that her mom is not great at putting things away and there are piles
of clothes everywhere around the house. ‘My mom hates throwing anything away”.
Mrs. Woods interrupts and says, “everything I have is perfectly good!” I can take
care of myself!”
Susan has noticed that her mom forgets to take her pills some mornings.
When asking Mrs. Woods about her bladder function, she admits to you that she
has a “little problem with leaking urine”, when she sneezes or coughs. Mrs. Woods
states that she gets a little forgetful when tired. “I get tired because I don’t sleep
well because I have to go to the bathroom a lot at night”.
Scenario 2
Mr. Klein is 45 years old and with a history of Crohn’s disease for the last 20 years.
He is married and has 2 teenager daughters. His family owns a small ranch which
requires long hours of physical activity daily. When working in the fields, he has limited
access to water and/or a washroom. Due to complications from his disease, Mr. Klein
recently had his large bowel removed and now has a permanent RUQ ileostomy. Prior
to surgery, Mr. Klein was very socially active, meeting friends for coffee daily and playing
cards weekly.
He was discharged 2 weeks ago and has come into the clinic for follow up. Mr. Klein
tells you that he can barely look at the stoma and hates emptying the pouch. “I leave
it until it is full before emptying”. “I hate this thing!”
He complains that the ostomy pouch falls off every day. Mr. Klein believes this is
happening because the stool is too watery. He describes the stool as pasty brown
the like watery oatmeal. I have started drinking less water and am taking Lomotil to
try to get is “more solid”.
His skin is very dry, lips are cracked, and skin turgor tenting. VS are T 37.1 , Pulse
104bpm weak and regular, BP 90/58 Resp 20 bpm. O2 97% RA
When speaking to Mrs. Klein, she tells you that her husband is not coping well,
refusing to talk to his wife about how he is feeling. Mr. Klein will not accept any
help with stoma care. He tells his wife that he believes the ileostomy will be
temporary and will be reversed.
Mrs. K tells you that her husband has a mild learning disability and has challenges
processing new information when verbally given. She states his temper is worse,
and he refuses to leave the farm to run errands or meet his friends for coffee.

Guidelines
1. Introduction
• Clearly identify your client, provide a client summary.
• Provide an outline describing what your teaching plan is about.
2. Assessment Data & Learning Needs
• Identify two learning needs for the client.
• Describe your rationale from the scenario for each learning need.
3. Priority Learning Need & Diagnosis
• From the two identified learning needs, clearly indicate which one learning
need is the highest priority for your client.
• The client learning need chosen must be the correct priority.
• Write a deficit knowledge nursing diagnosis for that one selected priority
need. (Your Nursing Diagnosis MUST start with the Nanda “Deficit
Knowledge”.
• Ensure the Diagnosis is in the correct format. (Nanda Diagnostic Statement,
Etiology, Evidence) (See Ackley textbook for correct format)
4. Goal Statement
• Write one patient centered goal for this priority learning need. (Goal
statement should be one brief generalized statement)
5. Learning Outcome
• Write one expected learning outcome using SMART Criteria (specific,
measurable, attainable, realistic, timely)
• Note: Learning outcome should be one statement that includes all
components of SMART criteria.
6. Domains of Learning
• Identify two domains of learning appropriate for your client that will best fit
the teaching method you wish to use for the identified priority learning need.
• Must relate to goal and learning outcome.
• Provide rationale for your choice.
• Include literature support for each.
7. Barriers of Learning
• Identify two client barriers presented in the scenario that may affect the
teaching/learning process.
• Include rationale for each barrier. (Why are they a barrier?)

8. Teaching Plan
• Outline 3 specific topics for the educational content of your teaching plan.
(What specific topics does the nurse want to teach the client?) Provide
details as to what exactly you are teaching for that topic.
▪ Topics should be based upon your chosen domains of learning.
▪ Education content should relate to expected outcome.
• Each educational topic identified must have one reference.
▪ References must be from 3 different sources. Two from academic
journals less than 7 years old. (NOTE: A website is not an
academic journal)
9. Teaching Methods
• Outline teaching methods to be utilized.
▪ Include setting of sessions
▪ Include timing of sessions
▪ Type of session
▪ Specific teaching strategies – provide details
10. Teaching Resource
• Include specific teaching resources (materials) that you would be using in
this teaching plan. (e.g., Specific video link, pamphlet) eg. If you are using a
video. What is the video?
• There should be 3 different resources. (One per topic)
• Ensure your teaching plan/methods are related to the domains of learning
you chose in step 6.
11. Evaluation of Learning
• Identify how the client with be evaluated in this specific teaching plan
based upon both chosen domains of learning. (Refer to Chapter 21 Potter
and Perry). Provide details
• Evaluate how the expected outcome has been achieved using each
domain of learning
• Provide literature support for the evaluation of each domain of learning
used.
12. Analysis of the Teaching Process
• Identify and discuss at least two reasons why patient teaching is important
in nursing care of this client.
13. Self-reflection
• Identify and discuss two strengths and two challenges in the
implementation of this specific teaching plan.
14. Conclusion
• Provide a concise conclusion, summarizing main points of the teaching plan
you developed.
• Discuss the benefits of developing a teaching plan.
• How do teaching plans benefit your future nursing practice.
