Synopsis of issue: Provide a concise overview of the issue identified in Part A and the health and wellbeing impact it has for older adults in their healthcare setting (e.g., acute, community, or palliative). 2. Priority goals: Identify two priority goals that address the issue and justify why they are a priority in this context. 3. Evidence-based actions: For each goal, provide three evidence-based actions that are relevant to RN Standards for Practice. Ensure that at least one action involves interprofessional collaboration and another demonstrates a person-centred approach. Support each action using relevant evidence. 4.
