At the beginning of the course, the course instructor will provide each student with a set of descriptors about a client that will serve as a basis to prepare a developmental case study/assessment paper throughout the course. A description of the requirements for each section of this assignment will be found in the course site in almost all of the modules and located in the Module Introduction Page. All of the modules , except for Module 1 and 11, will include an assignment requiring an additional section to be developed about their client in the case study assessment paper. It is recommended that the student build each section of their case study/assessment paper as they complete each module. The student will incorporate readings and module instructional materials, supplemental resources, interactive websites and their own research into the preparation of the case study/assessment paper. They will need to demonstrate that they have understood the course content and to integrate that content into the assessment report” of their client. Provide references to support the descriptions developed in each section.

Case Study Assessment Paper – Section 1 Provide your client’s full name, date of birth, client’s address, the name of your agency you work for, your title, source of referral and reason for referral. Provide a brief description of the client profile you selected such as age, gender identity, sexual orientation. Do not go into detail about your client’s mood altering, addictive behaviors yet. You will add that in the next two modules.
Case Study Assessment Paper – Section 2Provide a description of your client’s substance use practices including the specific type of substance (s) the person is using, how often, how it is being administered, how much, the effect of the substance, any side effects of the substance, how age, gender, size, cultural background affect the impact of the drug, how other prescribed medication may interact with the substances, etc. Describe past withdrawal attempts and their outcomes. Based on the information collected, determine what the withdrawal management plan could be for your client, if they chose to reduce or stop their use.

Describe any mood-altering behaviors your client may be involved with and how the behaviors interact with the substance use. What purpose and function do the substances and behaviors play in your client’s life? What may have been causative factors leading to your client developing an addiction? Using the PowerPoint material, describe the process of use/addiction your client experienced over time. What key characteristics of addiction are evident in your client’s life?
Case Study Assessment Paper – Section 4Describe the family relationships that your client is involved with. From the family members’ perspectives, what is the impact of addiction on those individuals? What roles do the family members play to find a balance to the family system? How does addiction challenge or fit into the cultural norms of the family? What is one benefit one family member may experience because of addiction within your client’s family? From your client’s
perspective, what is the impact of addiction on family relationships?.
Section 5 Identify 1 screening tool that would accommodate the needs of your client and provide references/sources. Describe how you would use it with your
client, how your client responds to the implementation, and how the client would score based on the information that you have so far.Review 2 sample videos from the Screening, Brief Intervention and Referral website from Boston University as provided in Module 6. Describe at least one thing from each of the two videos that you could use to help determine if your client’s use/mood altering requires further, more in depth assessment. How will you use those with your client?
Case Study Assessment Paper – Section 6Describe your client in more detail and depth than you did with Screening in the previous module. Use the following headings: Biological: Medical conditions and medications, weight, mobility, personal hygiene, name of medical supports, diet, sleep issues, activities of daily living (dressing, bathing, toileting, feeding)Psychological: Emotional state, past and present psychiatric symptoms and diagnosis, hospitalizations, suicidal history and current risk, trauma, name of psychiatrist or counselor, medications for psychiatric conditions.Social: Legal history and current involvement, hobbies and recreational pursuits, transportation and accessibility considerations, friends and family relationships and impact of addiction on them, employment history, financial state and money management , housingCulture: Cultural perspectives about addiction/help-seeking/recovery, cultural practices that may influence changeSpiritual: Faith involvement in the past and currently.
Client Goal: Identify the client’s goal about use and Recovery capital (resources currently in place, strengths and experiences client brings to assist in making changes). Impressions: What Stage of Change the social worker perceives the client is at and rationale for that choice, and lastly, list the items on the DSM V Addictions category that match with the client’s situation and experience.
Case study Assessment Paper: Section 7 Describe harm reduction strategies that would reduce the harms specific to your client’s situation. Identify how the family would respond to this plan and how would you address any barriers for carrying out the strategies that the family may pose.
Case Study Assessment Paper: Section 8 Identify at least one counseling model that would be effective with creating a client centered environment and engaging the client in the change process. Refer to at least 1 article that supports using the model(s) with your particular client. What issues or characteristics are present in your client’s context that fits with your choice of counseling model(s)? How will you engagethe family in the change process? Refer to at least 1 article that supports your plan for working with the family.
Case Study Assessment Paper: Section 9Describe what factors lead to relapse with your client and devise a relapse prevention plan that addresses each factor. Would you need to adapt the counseling model(s) that you identified in Section 8 of your paper to accommodate your client’s cultural needs? If so, how would you adapt it? What approach can you implement as a social worker to foster spiritual growth with your client? Identify a plan the client will carry out to engage in spiritual activities that will assist in recovery including the reference(s) for at least one article to provide a rationale for the choice of those activities.
Case Study Assessment Paper: Section 10: List the resources that your client has agreed to utilize for recovery and the tasks that you, as the counselor, and the client will need to carry out for a successful referral. Describe the rationale for the resources chosen for your client. Describe one approach that is discussed in the reading for this module that may fit with prevention that could have been implemented and may have been effective for early intervention with your client before the use progressed too far.
